3.0  System or Program Documentation and Manual

There shall be a documentation of the presence of the Maternal Death Control Management System or Program that has been presented and approved by the governing body or top management.   

The Maternal Death Control Management System shall be contained in a manual of operations, both in print and in digital media. 

A copy of the print manual shall be kept by the Maternal Death Control Management System Team. 

A copy of the digital manual shall be posted in the hospital intranet and/or internet site for easy accessibility and coordination. 

The Maternal Death Control Management System Manual of Operations shall be continually reviewed and updated at planned intervals at least once every 3 years and anytime revision is deemed necessary. 

The Maternal Death Control Management System Manual of Operations shall contain at least the following components of the Maternal Death Control Management System:

Goals and Objectives, Key Result Areas, and Key Performance Indicators

Cause Mapping

Design and Development Blueprints

Deployment, Education and Implementation Plans

Evaluation, Review and Continual Improvement Plans

Documentation and Archiving Plans

Management Review and Independent Audit Plans 

If needed, the Maternal Death Control Management System Manual of Operations shall be supplemented by another manual containing implementing guidelines and instructions. The latter shall be named Supplementary Manual on Implementing Guidelines and Instructions (SMIGI) of the Maternal Death Control Management System Manual of Operations.
