As a patient at ZAMBOANGA CITY MEDICAL CENTER, you have the following responsibilities:

Bilang pasyente sa ZAMBOANGA CITY MEDICAL CENTER, ikaw ay may mga tungkulin tulad ng mga sumusunod:

Know your rights.

“ Na alamin ang iyong mga karapatan”.

Provide accurate and complete information about  your health and medication, history, and allergies.

“Na ipaalam ang mga kaukulang impormasyon tungkol sa iyo at iyong dinaramdam”.

Inform authorized personnel immediately when you  notice any changes in your condition or disease.

“Na agad ipahiwatig ang mga napapansing  pagbabago sa nararamdaman ukol sa kalusugan  at karamdaman”.

Understand the reasons behind the treatment being performed and the necessary expenses required of them.

“Na unawain ang mga kadahilanan ng mga lunas na ginagawa at ang kaukulang gugulin para ito”.

To accept the results/effects of treatment performed for the reason that all have been explained to you from the start.

“Na tanggapin ang kinalabasan ng gamutan sa dahilang ang lahat ay naipaliwanag sa iyo nung una pa man”.

Fulfill your financial obligations by paying for services and care rendered by both the hospital and your doctors.

“Na gampanan ang tungkulin na bayaran ang mga dapat bayaran sa nangyaring gamutan at pag-aalaga”.

Be considerate and respectful of the rights of other  patients and hospital personnel.

“Na igalang ang mga manggagamot, lahat ng nangalaga sa iyong kapakanan at maging ang iyong kapwa pasyente”.

Perform your obligations to yourself.

”Na tuparin ang mga tungkulin mo sa iyong sarili”.

Participate and cooperate in all your obligations for the improvements of your health.

”Na sumali at tumulong sa lahat ng mga dapat mong gawin tungo sa pagbuti ng iyong kalusugan”.

Always respect this health facility and all personnel involved in its mission.

“Na palaging igalang ang pagamutang ito at ang lahat ng taong may kaugnayan sa misyon nito”.

Remain true to all discussions/conversations that you were part of.

“Na manatiling tapat sa lahat ng mga usapang kung saan ikaw ay naging bahagi”.

Responsible to follow hospital rules and regulations that affect patient care and conduct. 

 “Responsibilidad na sundin ang mga panuntunin at regulasyon sa hospital na makakaapekto sa pangangalaga at kaayusan ng mga pasyente”.

That all your health records are true and were not changed in any way, whether they were given or taken from ZCMC.

“Na ang lahat ng tala ng iyong kalusugan ay  pawang halaw sa katotohanan at walang  ginawang pagbabago sa anumang pagkakataon, ibinigay man o kinuha ang mga ito sa ZCMC”.

To cooperate with physicians in their training for expert treatment/care of children.

“Na makipagtulungan sa mga manggagamot  sa kanilang pagpapakadalubhasa sa  panggagamot/pag-aalaga sa mga kabataan”.

To inform to proper authorities of this hospital of all violations as perceived by you.

”Na ipagbigay alam sa mga kinauukulan sa pagamutang ito ang lahat ng napansing mga paglabag ayon sa iyong pananaw”.

PATIENT’S RIGHTS

We encourage you to speak openly with your health care provider, take part in your treatment choices, and protect your own safety by being well informed and involved in your care. As a patient at Zamboanga City Medical Center, you have the following rights:

1. You have the right to receive considerate, respectful and compassionate health care in a safe setting regardless of your age, sex, gender, religion, ethnicity, political affiliation, disability or capacity to pay free from all forms of abuse, neglect or ill treatment.

2. You have the right to be assigned to a competent doctor/resident physician and be told of the names of all health care team members who are qualified to provide diagnosis, treatment and medical advice. Likewise, you have the right to know your hospital and physician fees, and receive information about the possibility of financial assistance.

3. You have the right to notify a family member or person of your choice and your chosen doctor of your admission to the hospital.

4. You have the right to have someone remain with you during your hospital stay unless it compromises you or others’ rights, safety or health.

5. You have the right to exercise your spiritual and cultural beliefs within the capacity and rules of the hospital/medical center.

6. You have the right to be informed and give consent before any non-emergency procedure or research/experiment or to refuse such.

7. You have the right to privacy and confidentiality of your medical records according to laws, as well as in acre discussions, examinations, and treatments and the right to see or get a copy of your medical records except those records restricted by law.

8. You may request for an escort during physical examinations.

9. You have the right to be represented by someone (assignee) to decide on your behalf when the circumstances warrant.

10. You have the right to ask about and be informed of the complaint process and express grievances without fear of recrimination or reprisal. You are encouraged to speak directly to the health care provider involved in your care.

If there are issues not resolved to your satisfaction, or if you would like the help of someone not immediately involved, talk to the Public Affairs & Customer Care Unit Staff or to the Chief of Hospital.

If you find the above avenues unsatisfactory, you may choose to file a formal grievance with the following agencies:
